Below are the steps to schedule your first appointment and then prepare for your visit. This includes downloading the forms to fill out and what to expect when you arrive at one of our six health centers
As a Federally Qualified Health Center, WindRose Health accepts all people regardless of ability to pay and helps patients overcome language, economic, cultural and geographical barriers to obtaining health care services for themselves and for their families.

Step 3: Arrive 15 Minutes Early to Your Appointment
For your health and safety, be prepared for the following:
COVID screening
Your temperature will be taken
A new mask will be provided to you
Step 4: Check In With the Front Desk Receptionist
Provide your identification
Provide your insurance card
Step 5: Meet With Our Eligibility Navigators
Learn about our Windrose Health Network policy and resources available
Step 6: Meet Your New Primary Care Provider
Provide your new PCP with all the medications your currently taking
Discuss your health concerns with your PCP
Locations
WindRose Health Network launched in 1996 as a Rural Health Center based in Trafalgar, Indiana. Today, we have five locations from southern Indianapolis to Johnson and Bartholomew counties.
COVID-19 Update
Our Health Centers are open to in-person patient visits. For patient convenience and safety, we continue to offer Tele-Health Visits (if appropriate) for chronic health conditions and other medical issues. If you think you may have been exposed to COVID-19 or are having active COVID-19 symptoms, please call for medical advice before coming to your appointment.
